- Provides legal protection for the use, possession, and, in some cases, cultivation of cannabis for medical purposes
- Free (no county fee for San Francisco residents)
- Approximately two weeks
- Cards must be picked up in person
- Apply at the Office of Vital Records, located at:
San Francisco, CA 94102
- All visitors must sign in at the security desk on the 1st floor before proceeding to the 5th floor.
- A completed MMIC application
- An original letter from a licensed medical provider stating that medical cannabis is appropriate, issued within 30 days
- Proof of identity (CA Driver’s License, CA ID, or Passport)
- Proof of San Francisco residency (utility bill, photo ID, or recent rental/mortgage payment; P.O. Boxes not accepted)
- Patients may designate one primary caregiver on the application
- Caregivers must have a pre-existing relationship with the patient related to housing, health, or safety
- Caregivers must be 18 or older and must accompany the patient when applying
